Notes: A big thanks to the pediatrician scientist program for bringing in this week's speaker. I can say that she is teaching about something that I have not given a lot of thought to before—the intersection of the placenta and fetal brain development. Notes: I don't know if you have ever had the chance to talk to someone who works at poison control about their job, but if you get the chance, take it, because what they do is fascinating. There are so many CRAZY products out there that people are using (think of all of the stuff you can buy legally at gas stations and vape shops including synthetic cannabinoids and opioids, mushrooms, kratom and 7-OH). Notes: What can I say except that this grand rounds is just cool? It is not likely to change my day-to-day patient management, but as someone who followed the recent Artemis mission with glee, it is really fascinating to hear the physician's role in the space program. Dr. Kreykes covers how physicians contribute before, during (running experiments and handling medical emergencies in space--yikes!), and after space missions. We will be back at 19WT auditorium, and she is coming in person, so please come in person too to welcome her.

I'm sure I'll get some arguments here (let's fight it out!), but I cannot think of a class of drugs that emerged during my career with more potential to rock the medical world than GLP-1s. I know I'm excited to learn everything I can about how they work, who should be on them, how to prescribe them (so it gets covered), and what their broader impact may be. Notes: This week's Grand Rounds is Resident Scholarship Day—one of my personal favorites of the year! Each year, residency leadership selects three residents to present their research, and they always put together a diverse lineup. This year covers a lot of ground: basic science (intestinal microbiota), global health (TB-HIV in Uganda), and public health (drowning prevention). The residents have put serious time and effort into both their research and their presentations, and it really shows. Notes: In honor of Mental Health Awareness Month, the psychiatry and psychology departments are bringing us a lecture which I know is very relevant to everyone practicing hospital-based medicine right now—mental health boarding of patients with autism and other behavioral health concerns. One quote from the slide show that will not come as a surprise to many: "From 2020 to 2025, mental health boarding increased by 54% for youth with ASD" at TCH. We have a panel of experts in complimentary fields talking about how they are addressing this at TCH and offering guidance on management of these complex patients.

I would say that this lecture is perfect for those of us who want to be inspired by physicians who are finding success under resource-limited settings (and I feel like we could all use some inspiration these days). Dr. Sanders shares her story from Texas to Lesotho, including the challenges and triumphs along the way.
Notes: This week's Grand Rounds presentation is the starting lecture for the annual education retreat. Dr. Lingard presents an interesting way to look at the language we use when we supervise (or are being supervised). Notes: Most of the policy decisions that affect our ability to practice medicine, our reimbursement rates, and the daily lives of our patients are decided at a state level. Texas is extremely complex politically, and on top of that, the legislature only meets for about six months every odd year. In this grand rounds, we have leaders from TCH government relations and Texas Pediatric Society discussing how we navigate this huge, diverse, often amazing, and sometimes frustrating state. They will review the successes (there were some!) and areas for improvement for this past legislative session as it pertains to pediatrics.

In this presentation, Dr. Kasparian explores the trauma and fear children experience as a part of hospitalization and how that trauma response can impact their health for a lifetime. She presents a solution in Cincinnati Children's Hospital's Heart and MInd Wellbeing Center, which is an interdisciplinary program dedicated to improving mental health outcomes in pediatric and congenital heart disease across the lifespan. Thank you to everyone for hanging in there with technical issues as we transition to Teams. I have been working closely with IT specialists who have been there for every Grand Rounds to help troubleshoot, but sometimes things just go wrong anyway! Our best thought is that there are some incompatibility issues between TCH Teams and BCM Teams which made Teams freeze last week, so we are trying to account for that this week. As you know, BCM does not contract with Zoom anymore so that is not an option for now.
